Dear Parent/Guardian,

The Missouri Department of Health and Senior Services requires students to receive one dose of the meningococcal conjugate vaccine (MCV4) before entering the 12th grade.  The CDC recommends this vaccine be given at age 16.

For your convenience, the above vaccine will be offered to all students 16 years old & older, during a School Shot Clinic to be held at our school by the Barton County Health Department.  In addition to the above vaccine, the Human Papillomavirus (HPV) vaccine, will be offered as well.  It is given in three doses to children 15 to 18 years of age.  The HPV shot is recommended but NOT required for school attendance.

Please fill out and sign the shaded areas on the attached Immunization Consent & History form for each child.  Please note a few specific instructions:

  • If your child has Medicaid through an HMO, the Medicaid number and the HMO number is required on the Immunization Consent & History form
  • If your child has private insurance, fill out and sign the attached insurance information form in addition to the Immunization Consent & History form
  • If your child is uninsured, underinsured, or is Alaskan Native or Native American, only fill out the Immunization Consent & History form

If you have any questions, please call me or the Health Department at (417) 682-3363.  Please return completed forms to the school by Tuesday, January 28th. You may keep the “Vaccine Information Statements” for your records.

Dear Parent/Guardian:


The Miles for Smiles mobile dental unit will be coming to Lamar schools the week of August 26th – 30th & September 4th – 6th.  The program has specific guidelines that must be followed in order to qualify for dental care.  Students who (1) have Medicaid (2) have MO HealthNet or (3) who have NO dental insurance may be eligible for this program.  


If you are interested in this program for your student(s), please print off the below form, complete & sign.  Bring the form back to your building’s office as soon as possible.   If you qualify, Miles for Smiles staff will contact the Health Services Program at the school & we will send out further information.
